Getting Medical Document

To receive a recommendation for medical cannabis you must be assessed by a licensed health care practitioner. Once assessed, the practitioner will determine if you are a good candidate for medical cannabis treatment.  If you are approved they will issue a medical document.

Yes. 

Typically, the first medical document will be for approximately 3 months.  This allows you to test the treatment and ensure it’s working for you.  After that, you will need to return to renew your document and so the health care practitioner can evaluate how the treatment is working.  If the treatment is working, your medical document will typically be renewed for 6 months to 1 year.

If you start the process but realize you don’t have time to finish or complete the consultation, you can return when is convenient for you. Practitioners are available 7 days a week.

With your consent, your medical document will be forwarded to Starseed for your convenience. If you’d like a copy, please contact our Client Care Team at 1-844-756-7333.

Yes.

You can contact Starseed at any time to transfer your document to another licensed producer. 

Renewals

Starseed can receive Medical Documents through our online portal. Your Health Care Practitioner can submit your signed Medical Document for you by creating a Physician Account.

Alternatively, your Health Care Practitioner can submit your Medical Document through Sfax. Sfax is a secure fax that complies with Health Canada regulations. You can Sfax us at 1-844-756-0470.

While in the registration process, we do suggest making a copy of your Medical Document for your records in the event you’re approached by law enforcement.

To renew with Starseed, please log in to the Starseed Customer (FPO) Portal and review your Registration Application. Confirm that the information is still correct, specifically your full name, date of birth, address(es), and gender. If any of this information has changed, please print out and submit a new Registration Application. In addition, we will need your prescribing Health Care Practitioner to renew your Medical Document, which they can do through our online portal, if they are registered with us.

To continue qualifying for Starseed’s Assisted Pricing Program, we require new information upon your renewal, to ensure that your information is accurate and up-to-date.

Under the Health Canada regulations, we are only able to accept the original Medical Document. If you are in possession of the original, you can mail it to us at the following address:

Your Health Care Practitioner also can send us your Medical Document by secure fax, or online.

Once we successfully receive your Medical Document and any other paperwork that is required, you will be able to place your order within 1-2 business days. You will also receive email confirmation from Starseed once you are ready to reorder.
